Intake Paperwork: Prior to your first session, please complete the intake paperwork Tami has provided you (or CLICK HERE to submit a form to request the intake paperwork as well the release of information documents and Tami will get them to you) and bring payment for your session (check, cash or credit cards are accepted).

How to Prepare (optional): If you would like to prepare for the first session, you can think through the following questions and write out any notes you feel might be helpful.
- What motivated you to contact me for this first session?
- How would you describe the “issue” or challenge you are facing?
- What background information is important for me to understand?
- What are your goals and expectations for counseling?
